Disaster Case Management

CCCSDC has over 60 years of experience in providing services to low-income residents of Dutchess to stabilize housing, meet basic needs, and increase self-sufficiency. Our holistic Case Management Program has 4 cornerstones:

  • Individual Assessment. Clients that meet basic criteria, including residency and financial need, are assessed to determine the causes of their problem and their future ability to meet ongoing needs.
  • Service Plan. Clients and caseworkers formulate a service plan, which is a roadmap for navigating the current challenge and planning for the future.
  • Client Advocacy. Our staff works with the client’s utility provider / landlord etc., to develop an agreement that allows the client time to meet his/her arrears. Emergency relief is tied to the achievement of goals identified in clients’ service plans.
  • Ongoing Support. Our staff monitors clients’ progress to ensure ongoing stability. We also provide financial literacy classes for clients who receive relief.
